Request for a Government Record (UIPA Request)
1. How can I get a copy of the investigator’s report pertaining to a violation at a licensed premises?
After an adjudication hearing (public) on the violation has been completed, you may request a copy of the report by filing a Request for a Government Record (OIP-1). Investigator’s reports are not available prior to a public hearing.
2. How do I request for a history of violations or other information about a liquor establishment?
Complete the Request for a Government Record (OIP-1) and email it to liquor@honolulu.gov or mail it our office.
Annual Employee Listing
Annual Employee Listing (AEL) packets containing a current list (form) of all employees who are registered to your license, and an informational letter on how to complete and submit your AEL form will be mailed to your corporate address on file, before the beginning of your reporting period (please refer to the Rules of the Liquor Commission §3-82-38.8). Reporting periods are based on your license’ class and will begin on the first business day of each month.
Email (or mail) completed AEL to HLC@honolulu.gov by or before the posted deadline based on your liquor license class.
