- Required Signatures & Notarization:
- If there is a lienholder: The lienholder must sign the application.
- If no lienholder: All registered owners must sign the application.
- Notarization required if submitting application by mail.
Damaged/Mutilated Title: If the original title is damaged or defaced (rather than lost), you must submit the original with the application.

Send us a letter requesting a verification that a Hawaii title was not issued and include the following information:
- Name (as it appears on the registration)
- Current mailing address
- Vehicle Identification Number (VIN)
- License Plate
- Email address (to notify you of any discrepancy or incomplete paperwork – we do not save this information)
- Signature

Individuals transferring a vehicle to another state that requires proof of vehicle title should obtain a certified verification letter from the City and County of Honolulu DMV, ensuring the validity of the vehicle’s registration and title. Provide the certified letter to your local DMV, which will use it to process your new registration and title.
To request for a certified verification letter, contact the DMV and provide the following information:
- Name as it appears on title
- Vehicle Identification Number (VIN)
- License Plate
- Current mailing address or email (to send the certified verification)
- Reason: Indicate that you are requesting a certified letter verifying your vehicle’s title and registration.
