HONU (Homeless Outreach and Navigation for the Unsheltered)

(one full time vacancy and one part time vacancy)

Under the general supervision of the Community Services Specialist V, the HONU Community Services Specialist III is responsible for fulfilling the following job duties:

● Conduct on-site assessments, formulate care plans jointly with participants, connect to resources to assist with sheltering them, and identify supportive services.

● Monitor and maintain participant records, including but not limited to demographic information, needs, health conditions, interviews, contacts, journal notes, progress notes and other documentation needed.

● Is responsible for identifying and addressing barriers, making referrals to service providers and monitoring participant progress.

● Utilize person centered, motivational interviewing, mediation and other techniques.

● Identify and explain various service options, assist with linkages, and follow-up to ensure those linkages are implemented and appropriate.

● Collaborate with lower level specialists to ensure participants are prepared for their stay at HONU, complete assessments and care plans, receive appropriate services and link to housing options and community resources.

● Monitor progress of participants, provide additional support and make changes to care plans.

● Submit and maintain required documents, reports and participant records such as the Homeless Management Information System and other relevant data bases.

● Maintain and update a list of shelters and provide relevant information to the public, participants, agencies and staff as appropriate.

● Maintain cleanliness of the HONU site and perform other duties as required.

Minimum Qualification Requirements:

Training and Experience: A combination of education and experience substantially equivalent to graduation from an accredited college or university with specialization in psychology, social work, social sciences or a related field and two years of progressively responsible professional employment service, counseling, social work or related work experience.

Knowledge of: principles, methods and techniques relative to educational, employment and vocational guidance, counseling and testing, job development and employer relations, youth guidance, counseling and testing; services, activities and programs of youth, educational, vocational and related programs; standard aptitude and proficiency testing; local labor market and vocational training programs; local community resources for those in need of supportive services; report writing.

Ability to: understand and explain laws, rules and regulations; obtain, analyze and evaluate facts and to draw inferences and conclusions; explain the functions of the pertinent program as well as other available programs of the department; assess, analyze, develop and monitor individual service plans; analyze and explain to others the use of the results of proficiency and aptitude tests; deal effectively with people to promote the services of the program; prepare clear and concise reports.
