I. Program Activities
Respond to crisis calls, gather initial information and create a case management file for further review by the CORE Field Supervisor. Fine tunes skills by learning various styles and methods to perform case management duties from direct training to provide one-to-one assistance and/or group assistance. Fine tunes case management skills to chart the participants’ progress, setbacks, adjustments and achievements. Conducts in-depth assessments to determine participant needs. Participates in on-site monitoring and follow-up of assigned cases for additional evaluation. Keeps detail records for reports and monitoring purposes. Under direct guidance and supervision of the CORE Field Supervisor, collaborates with community resources and participates in case reviews. Assist in developing support plans for each participant. Modify support plans as needed based on any changes in needs and services. Explain program rules and service options to participants.
II. Administrative/Other Program Activities
Under close guidance and supervision of the CORE Field Supervisor, assist in preliminary and implementation of the CORE Program. Assist in identifying resources while maintaining relationships with government and community agencies and businesses to ensure resources and services are available and implemented on a regular basis. Performs other related duties as assigned.
