SERVER-TRAINING PROGRAM - BARTENDERS OR MANAGERS
MONDAYS, TUESDAYS & WEDNESDAYS.
The Liquor Commission's Server Training Class - for bartenders or authorized managers/assistant managers - is conducted three times a week, Mondays, Tuesdays, and Wednesdays (see "*Exceptions*", if any, below) from 8:30 a.m.-11:30 a.m. Seating is limited to the first 48 applicants. Also, please call (808) 768-7355 or (808) 768-7359 to confirm dates & times, and to obtain parking info - we do not validate parking.

- TB clearance from any authorized medical facility, to include military, personal physician or other similar facility, verifying a 0-9 mm negative reading, or X-ray clearance.
- A curent government-issued identification card (Driver's License with photo, any State-issued ID card with photo, Military ID card, Passport).
- Proof of Social Security number.
- MANAGERS and ASSISTANT MANAGERS ONLY: A signed letter from the employer, authorizing registration.
- *$10.00 CASH for each establishment you are registering for.

*EMPLOYMENT OF A MINOR UNDER THE AGE OF 18 IN A LIQUOR ESTABLISHMENT
(The Minor must be at least 16 years old and cannot sell or serve liquor unless in a certified work program.)
*REQUIRED DOCUMENTS FOR SUBMISSION BY THE LICENSEE:
1. A request from the Licensee (hereafter referred to as the Employer") stating the exact position the Minor will be holding, the Minor's proposed work schedule, and a contact phone number(s) for the Employer.
2. A written letter of consent from the Minor's parent or guardian, including the contact home and work phone number(s) of the parent/guardian and a contact phone number for the minor.
3. A written letter on the school's letterhead from a school official attesting to the satisfactory academic grade standing of the Minor; or a copy of the graduation certificate/diploma and the school official's contact phone number(s).
4. A copy of the following: a) "Hawaii Certificate of Age, Wage and Standards" card issued by the State of Hawaii's Department of Labor & Industrial Relations-Wage Standards Division-Child Labor, and b) Minor's photo Identification (ID) Card - only the following current, valid IDs will be accepted: Driver's License, State ID, Military ID, or Passport.
?The request will be processed only upon receipt of ALL the above listed documents.
?The Employer will be notified by U.S. mail in approximately three (3) weeks.
?It is then the Employer's responsibility to notify the Minor and ensure the Minor is properly registered in a timely manner.
