Street Usage Application Forms

The forms listed below are provided in Adobe Acrobat Portable Document Format (PDF) and require the Adobe Acrobat Reader for viewing and printing. This is for informational use only.  Actual forms can be obtained at our office, 2nd floor, Frank F. Fasi Municipal Building, 650 S. King St.  Please do not submit forms listed below to our office.

1. Submit completed Banner application to Traffic Signals and Technology Division, 2nd Floor, Frank F. Fasi Municipal Building, 650 S. King St. Honolulu, Hawaii 96813.  Phone (808)523-4174; Fax (808)523-4446.

2. Submit art design of the banner to The Mayor's Office of Culture and the Arts for approval. Phone (808)523-4674 to schedule arrangements; Fax (808)527-5445.

3. Submit Culture & Arts approved banners to the Department of Facility Maintenance for installation. Phone (808)527-5923 to schedule job and pay fees ; Fax (808)527-5020.
