If your identification card or Electronic Benefits Transfer (EBT) card is damaged or destroyed, notify the EBT office by contacting the Eligibility Worker assigned to your unit.  Contact phone numbers are located in the local telephone directory.  This will help eliminate problems regarding check cashing, medical aid, or food assistance.
